This COVID-19 self assessment tool will indicate whether it is safe for you to be on campus today. After you answer the questions, you will receive a message either clearing you to come to campus or instructing you to remain off campus and explaining what next steps to take.
The information is confidential and is shared only with the Infection Control Officer, Infection Control Physician, Human Resources, and your manager, except when it is necessary to trace exposure to identified cases of COVID-19.

If your schedule doesn't conform to the above schedule please stop in anytime on M/Tu and Th/F
To assist with the process and move through the testing quickly, always order your test the morning of the test.
Fevers, chills, body aches, fatigue, headache and nausea are symptoms that are expected to follow vaccination.
If you are experiencing such symptoms, and you completed your vaccination in the prior 72 hours, you do not need to contact the Infection Control Physician.

Do not come to Campus and self-isolate until cleared to come back to work.

You are not cleared to come to Campus. Please call the COVID-19 hotline [555-555-5555]. Tell them your symptoms or situation and that you work at a medical facility that is a congregate care setting. This will identify your situation as having priority. They will probably schedule you for testing at the hospital. If you live elsewhere you should make contact with the comparable public health service there.
